1. ELIGIBILITY: Sweepstakes (“Sweepstakes”) open only to legal residents of the United States (excluding Puerto Rico), age 18 or older as of March 10, 2009. Void in Puerto Rico and where prohibited. All federal, state and local laws and regulations apply. Employees, officers, directors and agents of Starz Media, LLC and their respective parents, subsidiaries and affiliates, service providers, suppliers, advertising and promotion agencies and any others associated with this Sweepstakes (and members of their immediate families) are not eligible to participate. Sweepstakes begins on or about 12:00:01AM EDT March 10, 2009 and ends 11:59:59 PM EDT April 26, 2009 (Sweepstakes Period).
   
   
2. TO ENTER: Go to www.chromeskullvictims.com complete the registration form to become a member and click the Submit button. Entries will not be valid until received by Sponsor. All Internet entries must be received and recorded by Sponsor’s computer (the official Sweepstakes time keeping device) by 11:59:59 PM EDT on April 26, 2009. When online, you will need to enter your full name, address, city, state, zip code, daytime phone number, e-mail address and age. For details on how your information may be used, see Sponsor’s terms of use/privacy policy at http://www.starzmediadigital.com/privacy. No robotic, programmed, script, macro or other automated entries, or third party entries, (collectively “automated entries”), all of which will be disqualified. Entries will be disqualified if they are altered, delayed, deleted, destroyed, forged, fraudulent, improperly accessed, inaccurate, incomplete, interrupted, late, lost, misdirected, non-delivered, stolen, tampered with, unauthorized or unintelligible.
   
   There is a limit of one entry per person/e-mail address/household during the Sweepstakes Period. All entry information submitted becomes the property of Sponsor and will not be returned. Should multiple names be listed on a single subscription/account, the first name listed will be deemed the subscription/account holder. Entries will not be returned or acknowledged. By participating, entrants agree to be bound by these Official Rules and decisions of Sponsor, which are final and binding in all matters relating to this Sweepstakes.
   
   
3. PRIZES/ODDS: Grand Prize (1): $2,500.00; First Prize (1): 10-pack of DVDs from Anchor Bay Entertainment (titles determined by Sponsor) (ARV: $160). Second Prize (1): Autographed Laid to Rest Poster. Total ARV of all prizes: $2,660.00. Odds of winning depend on the number of eligible entries. All prizes will be awarded provided a sufficient number of eligible entries are received. Taxes on prizes, if any, and any other costs not listed herein are the winner’s responsibility. Prize may not be assigned or transferred. No substitutions or exchange for cash allowed, except at Sponsor’s sole discretion. Prize is awarded “as is” with no warranty or guarantee, either express or implied.
   
   
4. PRIZE DRAWING: On or about May 3, 2009 a random drawing will be held from all eligible entries to determine potential winners. Potential winners will be notified by mail, e-mail or telephone within 5 days of the drawing date. Potential winners must complete, sign and return an Affidavit of Eligibility, Liability and (where permitted) Publicity Release within 10 days of notification, or prize may be forfeited. Any prize or prize notification returned as undeliverable or refused will result in entrant disqualification, and an alternate winner may be selected. If winner is a minor in his/her state of residence, the prize will be awarded to the minor’s parent or legal guardian and the parent or legal guardian must complete, sign and return an Affidavit of Eligibility and Liability and (where permitted) Publicity Release within the same time period. By accepting a prize, winner consents to the use of his/her name, likeness, photograph, biographical information and statements by Sponsor in advertising, promotional materials and on their web sites without notice or compensation. Limit of one prize per person/household/family.
   
   
5. GENERAL RULES: By entering, each entrant and winner assumes all liability for any injury or damages caused or claimed to be caused by the Sweepstakes, his or her participation in the Sweepstakes and/or the acceptance, possession, use or misuse of any prize, and releases and holds harmless Starz Media, LLC and its parent companies, subsidiaries, affiliates and the officers, directors, employees, shareholders and agents (collectively, Releasees) of each from and against any and all damages, including, without limitation, negligence, claims, actions and/or liability in connection therewith including, without limitation, personal and bodily injury, death or disability, property damage or loss or damage of any other kind. Entries are void if defective, altered, forged or submitted via unauthorized channels, and Sponsor reserves the right to disqualify any individual using such method. Starz Media, LLC and its parent companies, subsidiaries, affiliates and their respective officers, directors, employees, shareholders and agents are not responsible for any: (a) telephone, computer or network malfunction, human or other error or failure to properly process entries; (b) communication disruption or other forces beyond their reasonable control, including inability to access any web site, submit entries or other disruption related to internet traffic, virus, bug or unauthorized intervention; (c) damage caused by computer virus or otherwise resulting to any computer from entrant’s access to the Sweepstakes web site or entry into the Sweepstakes; or (d) printing or other errors appearing in the rules or other sweepstakes materials. If any circumstance occurs which impairs the administration, security, fairness, integrity or proper play of the Sweepstakes, Sponsor may, in its sole discretion, suspend or terminate the Sweepstakes by posting a notice on www.chromeskullvictims.com. If the Sweepstakes is terminated before the end date, Sponsor may select potential winner in a random drawing from all eligible entries received as of the termination date. Persons found tampering with or abusing any aspect of this Sweepstakes, or whom Sponsor believes to be impairing the administration, security, fairness, integrity or proper play of the Sweepstakes will be disqualified.
   
   
6. LIMITATIONS OF LIABILITY: Releasees are not responsible for misdirected or non-delivered entries/e-mails; or for lost, interrupted or unavailable satellite, network, server, Internet Service Provider (ISP), Web site or other connections availability, accessibility or traffic congestion, or miscommunications, or failed computer, network, telephone, satellite, or cable hardware, software or lines, or technical failure, or jumbled, scrambled, delayed, or misdirected transmissions, or computer hardware or software malfunctions, failures or difficulties, or other errors of any kind whether human, mechanical, electronic or network, or the incorrect or inaccurate capture of entry or other information or the failure to capture, or loss of, any such information. Releasees are not responsible for any incorrect or inaccurate information, whether caused by Web site users, or by any equipment or programming associated with or utilized in this Sweepstakes and assume no responsibility for any error, omission, interruption, deletion, defect or delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or tampering with or hacking of Web site. Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds to be tampering with the entry process or operation of the Sweepstakes or Web site, to be acting in violation of the terms of the Web site or to be acting in a non-sportsmanlike or disruptive manner, or with intent to threaten, abuse or harass any other person. Releasees are not responsible for injury or damage to participants’ or any other person’s computer related to or resulting from participating in this Sweepstakes or downloading or copying materials from or use of Web site. If, for any reason, Sweepstakes is not capable of running as planned by reason of infection by computer virus, worms, bugs, tampering, hacking, unauthorized intervention, fraud, technical failures or any other causes which, in sole opinion of the Sponsor, corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, Sponsor reserves the right, at its sole discretion, to cancel, terminate, modify or suspend Sweepstakes, and determine winner from entries received prior to action taken, or as otherwise deemed fair and equitable by Sponsor. Any attempt to deliberately damage any web site or undermine the legitimate operation of the Sweepstakes is unlawful and subject to legal action by Sponsor or its agents. In case of dispute regarding Internet entries, the authorized subscriber of email account used to enter the Sweepstakes, at actual time of entry, will be deemed to be participant and must comply with these Official Rules. Authorized account subscriber is deemed to be natural person who is assigned an email address by an Internet Access Provider, online service provider, or other organization that is responsible for assigning email addresses.
